All drivers sooner or later face the task of obtaining a medical certificate. A medical certificate for a driver's license is a mandatory document, without which it is impossible to obtain or renew a driver's license, with the exception of certain cases. Based on a medical certificate, a conclusion is made about whether a particular citizen can be allowed to drive a car. Medical certificate for driver's license

Documents required to obtain a medical certificate for drivers
In order for the driver to obtain a medical certificate, you must first prepare a list of documents. These include the following.
- Identity card (passport).
- Driver's license (if available).
- Military ID (if the citizen is liable for military service).
With these documents, the citizen needs to contact the registry of the institution that issues medical certificates. We repeat - he must have a license. If necessary, doctors may prescribe additional examinations and studies. It is possible to undergo a medical examination in another city, not at the place of registration, with the exception of a narcologist and a psychiatrist. Driver's medical certificate - why is it needed?
It is mandatory for drivers or driving school graduates to obtain a medical certificate confirming the absence of contraindications and restrictions for driving vehicles (vehicles) in order to ensure the safety of road users.
A certificate is issued to a citizen by a medical institution for submission to the State Traffic Inspectorate. In many cases, it is necessary to obtain a driver's license. Where can I get a medical examination?

Advantages of the state clinic:
- You can get a medical examination for free, although you still have to pay for the services of some doctors (this was discussed above), in addition, the tests are also not free.
Minuses:
- long queues in government institutions are still relevant;
- preliminary appointment with specialists, which can last for several months.
In private clinics, too, not everything is as smooth as it seems, although there are obvious advantages:
- no need to make an appointment with a doctor;
- there are no queues – the medical examination procedure can be completed easily, quickly and calmly.
But there are also significant disadvantages:
- an impressive price for services, considering that you will have to go through several doctors and undergo tests;
- A certificate from a narcologist and a psychiatrist can only be obtained in government institutions and only for a fee. There is no such service in private clinics.
How to get a driver's license?
There are several ways to pass a driver's test to obtain a certificate.
- The main one is under the compulsory medical insurance policy in a city medical institution. Most often, certain days and appointment times are allocated for this. But, as is known from practice, drivers still have to stand in line for some time and wait for a doctor to be called.
- The second method involves undergoing medical examination. commissions are much faster. In this case, you will be able to avoid running around on different floors, however, such a certificate is issued on a paid basis and is more expensive.
In any case, the selected medical institution must have a license for medical services, including permission to work in the field of neurology, ophthalmology, otolaryngology, therapy, functional diagnostics, and family medicine.
Not everyone knows how to get a conclusion from a narcologist and psychiatrist. This is allowed to be done only at the place of registration of the candidate driver. A doctor from a private clinic will not be able to issue a conclusion for a driver’s license. Procedure for passing a medical commission
The procedure for passing a medical commission to obtain a medical certificate for a driver's license is as follows.

Is a driver's medical examination required to replace a driver's license in 2021? Do new rules, and perhaps a new law, regulate this aspect? No, in accordance with the current order of the Ministry of Health dated June 15, 2015 No. 344n “On conducting mandatory medical examination of vehicle drivers of candidate vehicle drivers”, when replacing a driver’s license, if a person changes his personal data, or his the old document was lost or worn out, and if he receives additional rights of an international form, then passing a medical examination is not necessary for him.

After all, not so long ago, a commission when replacing a driver’s license was mandatory when entering into a registered marriage and changing the surname. How much does certificate 086 cost?
To date, this procedure has not been regulated at the federal level, so institutions resolve fee issues on their own.
- In public clinics, the document is issued both for money and for free. Some government agencies do not charge a fee for issuing a certificate; in others, the document is paid, but does not exceed 2,500 rubles. This is due to the fact that doctors have long queues and it can take several days to complete a medical examination.
- Private clinics can charge up to 15,000 rubles for a service if a full examination is required. But here the procedure goes quickly, and the quality of service is at a high level.
There are companies that offer, for a small fee, to issue a certificate 086 without undergoing a medical examination. You should not cooperate with such companies, since their activities are illegal. The received certificate will be invalid.
Cases when a medical certificate is not issued
There are cases when a citizen is denied a medical certificate. As a rule, this happens on the basis of the conclusion of doctors performing a medical examination, during which medical contraindications were identified. They are expressed in the presence of severe forms of illness in the driver. The Ministry of Health lists such diseases as:
- severe form of blindness;
- color vision impairment;
- diabetes;
- epilepsy;
- people registered with a narcologist and a psychiatrist;
- Parkinson's disease;
- mental illnesses and others.
Citizens suffering from the listed ailments are a source of danger not only for people on the road, but also for themselves. Therefore, they are prohibited from driving.
More information about the electronic form of documents
If a medical organization carries out electronic document management, then the following must be taken into account.
Electronic certificates and medical reports are issued upon personal application by the patient or his legal representative or when generating an electronic request (clause 5 of Order No. 972n). The request of the patient or his legal representative is signed with an enhanced qualified electronic signature or a simple electronic signature (provided that identification or authentication is carried out using a single identification and authentication) (clause 5 of Order No. 972n).
Note that the provisions of Order No. 972n on electronic form apply to all types of certificates and medical reports, including death certificates.
At the same time, the generation of electronic certificates, medical reports, providing the patient with access to them, as well as their issuance to the patient must be carried out in accordance with the requirements of the new Order 947n on electronic document management, a reference to which is contained in Order No. 972n (clause 16). It should be noted that in this standard, for unknown reasons, the regulator only mentions the patient, losing sight of his possible representatives, including legal ones. Moreover, Order No. 972n does not contain any reservations regarding access of close and other relatives to the electronic death certificate. Thus, it remains unclear how and on what basis these documents can be issued to the patient’s representatives, and an electronic death certificate – to the persons specified in clause 9 of Order No. 972n. It is interesting to note that legal representatives have had access to a number of government services for quite some time.
The next norm also raised questions. Thus, according to clause 18 of Order No. 972n, if a medical organization generates medical documentation in the form of electronic documents in accordance with the procedure for organizing a document flow system in the field of health protection in terms of maintaining medical records in the form of electronic documents, approved by the Ministry of Health of Russia in accordance with clause 11 of part 2 of article 14 of the Federal Law of November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in the Russian Federation”, a medical organization issues, at the request of the patient in accordance with this Procedure (ps: The procedure for issuing certificates and opinions), copies of certificates and medical conclusions on paper. Thus, the patient has the right not only to receive a certificate and (or) medical report in electronic form, but also copies of these documents on paper. However, this point needs clarification.
Firstly, the procedure for issuing these copies is not clear. According to paragraph 18 of the Procedure for issuing certificates and opinions in this matter, one should be guided by it, however, this procedure does not contain any rules for issuing copies of documents on paper, including copies of the electronic version on paper. In this case, it would be more reasonable to be guided by the Electronic Document Flow Procedure, which provides for provisions on the issuance of electronic documents on paper (Section VI), however, for unknown reasons, the regulator decided not to refer to Order No. 947n in this part.
Secondly, the source of funding is not indicated. However, we believe that one should be guided by the norm of the Procedure for Electronic Document Management (a copy of the electronic medical document on paper at the request of the patient or government agency is made at the expense of the medical organization (clause 24 of Procedure No. 947n)), although again there is a direct reference to this norm in the Order No. 972n is missing.
Thirdly, the legal representative was again excluded from the provision of a copy of an electronic document on paper. Moreover, if the question concerns a copy of the electronic death certificate, then in this case the norm is “not enough participation” of the persons listed in clause 9 of Order No. 972n (spouse, close and other relatives).
Validity period for a medical certificate for a driver's license
A medical certificate for a driver's license is valid for 12 months. Previously, this period was 2 years. But now, due to the tightening of legislative standards for drivers, the validity period of the document has been reduced. By the way, previously a photograph of the car owner was pasted onto these certificates - now this is not required.
The medical certificate is valid throughout Russia.
Responsibility for the absence of a medical certificate or the expiration of its validity period implies fines in the amount of five to fifteen thousand rubles. In this case, the car is taken away by a tow truck.
If a citizen decides to buy a certificate, then the following penalties apply to him:
- a fine of 80 thousand rubles;
- 480 hours of compulsory work;
- 2 years of correctional labor;
- arrest for a period of six months.
For falsifying a medical certificate, a citizen faces:
- restriction of freedom for a period of 2 - 3 years;
- compulsory work for two years;
- arrest for six months.
Specialists who issued a fake certificate are also required to bear responsibility, namely:
- be punished in a correctional facility for two years;
- perform compulsory work for two years;
- be under arrest for six months.
The authenticity of the medical certificate is usually checked by traffic police officers. This is very easy to do. It is enough to call the institution that issued the document and find out everything. In addition, it is mandatory to check the medical certificate of disabled drivers (their cars must have a special sign with the image of a disabled person) and for citizens whose driver’s license is marked “Certificate required.”
New form of medical certificate for license No. 003-В/у
A medical report in form No. 003-B/u has been used since July 1, 2016 (clause 4 of Order No. 344n). The new form of medical certificate is more informative and takes into account changes that have occurred in legislation since the introduction of the previous form.
New vehicle categories and subcategories
The certificate reflects the changes made to Article 25 of the Federal Law “On Road Safety” No. 196-FZ of December 10, 1995 (hereinafter referred to as Federal Law No. 196) regarding the introduction of new categories and subcategories of vehicles:
"A", "B", "C", "D", "BE", "CE", "DE", "Tm", "Tb", "M", "A1", "B1", "C1" ", "D1", "C1E", "D1E".
Now, for all new categories and subcategories of vehicles for which a certificate is issued, there is a separate field for marking.
Previously, the uniform certificate of 2010 provided for only 6 categories of vehicles for which a certificate was issued: “A”, “B”, “C”, “D”, “E”, trolleybus/tram (without category letter designation).

New information about driver health
Also, the new form takes into account the requirements of the Government of the Russian Federation No. 1604 of December 29, 2014, which establishes medical:
- Contraindications to driving a vehicle.

The new certificate contains corresponding tabular sections with several lines (types of contraindications, indications, restrictions), in the presence of which marks are placed in adjacent fields of the tables.
Previously, in the uniform medical certificate of 2010, only contraindications for driving a vehicle were indicated, as well as indications for special conditions for driving a vehicle (for persons with health limitations), which were entered manually in the “special notes” section.

Driver registration location
The certificate previously indicated only the driver’s place of residence. However, the law requires that examination by a psychiatrist and narcologist take place in psychoneurological and narcological dispensaries (offices) at the place of residence or place of stay (paragraph 3, paragraph 7, article 23 of Federal Law No. 196).
Therefore, a contradiction arose if the driver’s place of residence indicated in the certificate did not coincide with the place of examination at dispensaries at the place of stay.
In the new form, this contradiction is eliminated, since a citizen can indicate the place of his registration, both at the address of his place of residence and at the address of temporary stay.

What else has changed
The changes affected most sections of the medical certificate. Let's look at them separately.
Driver photo excluded
In the medical report on form No. 003-В/у there is no place for pasting a photograph, which existed in previous forms.
Drawing up a medical report without the need to take a special photograph for it should have a positive impact on the time and cost of its preparation.
Fields for signatures of passed specialists are excluded
Previously, a medical certificate was issued with the signatures of members of the medical commission and with the appropriate seal (of a doctor or a medical institution). These doctors were not always on site all at the same time, which caused a delay in obtaining the certificate.
The new medical report is individually drawn up and signed by a general practitioner or general practitioner (family doctor), and only the seal of the medical institution is affixed to it, which significantly speeds up its registration.
Notes from medical specialists excluded
Visits to the required specialists, of course, are not cancelled. However, the preparation of a medical report does not require, as previously, the collection of medical reports in a medical certificate. This innovation should also make life easier for the driver.
Previously, in medical certificates, the texts of medical specialists, their names, signatures, as well as dates, seals and stamps were not always distinguishable or could contain typos.
The absence in the new form of this data, which was previously filled out manually, should significantly reduce the risk of claims against the medical report from the State Traffic Inspectorate.
Important! The State Traffic Inspectorate believes that the changes will lead to an increase in the number of fake medical reports (now only one therapist’s signature and seal need to be forged) or to an increase in the number of dubious medical institutions that will issue a document without the driver undergoing all the necessary examinations.
Different surveys for different categories of vehicles
Clause 6 of the Procedure for Conducting Mandatory Medical Examination, approved by Order No. 344n, establishes a list of examinations by medical specialists, as well as a list of instrumental and laboratory tests that must be completed.
As before, examination by the following doctors is mandatory for all drivers:
- Therapist or general practitioner (family doctor).
- Ophthalmologist.
- Psychiatrist.
- Psychiatrist-narcologist.
For all drivers, when a psychiatrist-narcologist identifies symptoms and syndromes of diseases (conditions) that are a contraindication to driving a vehicle, a qualitative and quantitative determination of the presence of:
- Psychoactive substances in urine.
- Carbohydrate-deficient transferrin (CDT) in serum.
In addition, to obtain rights of categories “C”, “D”, “CE”, “DE”, “Tm”, “Tb”, as well as subcategories “C1”, “D1”, “C1E”, “D1E”, compulsory passage:
- Otorhinolaryngologist.
- Neurologist.
- Electroencephalography.
Also, according to appropriate indications, the following examinations may be carried out:
- Examination by a neurologist - on the direction of a therapist only if signs of contraindications, indications, or restrictions for driving a vehicle are identified.
- Examination by a surgeon - at the direction of a therapist only if signs of contraindications, indications, or restrictions for driving a vehicle are identified.
- Electroencephalography (EEG) - in the direction of a neurologist only if signs of contraindications to driving the vehicle are identified.

Previously, to obtain a medical certificate, an examination by an otolaryngologist, a neurologist, and a surgeon was mandatory for everyone.
Thus, Order No. 344n has reduced the number of doctors and examinations that drivers of vehicles of categories “M”, “A”, “B”, “BE”, subcategories “A1”, “B1” must undergo to obtain a medical certificate.
Validity period of the medical certificate
In this case, the change is not in favor of the driver.
Clause 12 of the Procedure for conducting a mandatory medical examination, approved by Order No. 344n, stipulates that the conclusion is valid for presentation to the traffic police departments of the Ministry of Internal Affairs of the Russian Federation within 12 months from the date of issue.
Previously, the validity period of a medical certificate was determined in accordance with Order of the Ministry of Health and Social Development of Russia dated April 12, 2011 No. 302n and was 1 year for drivers under 21 years of age, and 2 years for others (clause 17 of Appendix No. 3 and clause 27 of Appendix No. 2 to order No. 302n).
These validity periods for medical certificates (1 year and 2 years) have been repeatedly recognized by the courts in disputes between citizens and the State Traffic Inspectorate, when drivers considered the validity period of the certificate to be equal to the validity period of the rights issued on its basis (10 years).

What does form No. 003-В/у look like?
The medical report form, like medical certificates, is a security printed product of level “B”.
The form is made on regime paper and must contain:
- Water marks.
- Protective visible fibers of two types.
- Overlay of two background grids with variable pitch and iris roll (smooth transition from one color to another).
- Other means of protection against counterfeiting.
Form numbers (number ranges) can be issued centrally to government medical institutions or determined by the medical institution itself or the printing house.
Information about the medical institution can be entered into the form during production in a printing house, reprinted on a printer, executed in the form of a stamp, or entered manually. The form indicates:
- Name of the medical organization, OGRN code.
- Postal address and telephone number.
- License for medical activities: number, date of issue and validity period.
Regional differences in the new form and how much it costs
Requirements for forms used in state medical institutions are established not at the federal, but at the regional level. Therefore, forms may differ within different regions.
For example, in some places they are printed on two pages, which can be folded into a booklet, and in others on one single page. The color of the form and its design may also differ, depending on the different medical institutions and printing houses producing the form.
As an example, we can cite the requirements for this form established by Order of the Moscow Department of Health No. 568 dated June 29, 2016, according to which the format of the form has been reduced in comparison with the previous form. Currently, the size of the medical report is 148 x 210 mm (A5 format).
The cost of the form No. 003-В/у itself in the printing house is usually in the range of 10 - 30 rubles. But prices can be higher, depending on the place of purchase.
Obtaining a medical report
To obtain a medical certificate, a driver or a driving school graduate must apply with a passport (and sometimes other documents) to an institution that has a medical license to provide services for:
- Medical examination to determine the presence of medical contraindications to driving.
- Otorhinolaryngology.
- Ophthalmology.
- Neurology.
- Functional diagnostics.
- Internal medicine or general medical practice (family medicine).
At the registry, the citizen will be given a medical report form, in which, based on his passport data, the following will be entered:
- FULL NAME.
- Date of Birth.
- Place of registration.
The form is often issued after pre-payment for a medical examination, the cost of which depends on the categories of vehicles required for the driver and the region of the medical examination.
The minimum price varies:
- From 500 rub. – 1,500 rub. – for categories “M”, “A”, “B”, “BE”, subcategories “A1”, “B1”.
- From 1,500 rub. – 3,000 rub. – for other categories and subcategories of vehicles.
The driver will also be given a medical card to be filled out by medical specialists during the medical examination (clause 7 of Appendix No. 1 to Order No. 344n).
Depending on the medical facility and the specific situation, you may also need:
- Take blood and urine tests (as directed by your physician).
- Get an EEG, ECG (as directed by your therapist).
- Undergo a medical examination by a psychiatrist and a psychiatrist-narcologist in psychoneurological and narcological dispensaries, if the medical institution does not have such rooms.

How to fill out form No. 003-В/у + sample
The rest of the medical report is usually filled out by a therapist, based on the opinions of medical specialists.
It is allowed to make entries in form No. 003-В/у with a ballpoint pen with blue, violet and black ink (or ink of the same colors), as well as using printing devices.
- In the “Medical report” line, the presence or absence of medical contraindications, indications or restrictions for driving the vehicle is indicated using an underline.

Errors and corrections in the form
If a driver or traffic police officer discovers a blot, a spelling error, or a semantic error in the medical report, it may need to be corrected. Making corrections to form No. 003-В/у is not regulated by Order No. 344n.
Therefore, this issue will be resolved individually with representatives of the medical institution. Making corrections to forms of medical secure printed products (for example, certificates of incapacity for work) by employees of medical institutions issuing and processing them is, as a rule, not allowed.
Therefore, most likely, it will be necessary to reissue the conclusion on a new form and hand over the damaged copy for destruction. Whether in such cases there will be an additional charge for a new form and for the GP to reissue it depends on the specific situation.
In any case, a repeat medical examination should not be required, since all the information necessary to draw up a conclusion is indicated in the medical record, a damaged copy, or a citizen’s passport.
Corrections in the form by the driver himself
What you definitely shouldn’t do is try to make corrections to form No. 003-В/у yourself. In judicial practice, it is not uncommon for drivers to change the date of issue of an expired medical certificate to a later date.
Such actions can be qualified under Part 1 of Art. 327 of the Criminal Code of the Russian Federation, as a forgery of an official document granting the right to issue a driver’s license.
In addition, the very presentation of a fake certificate to a traffic police official qualifies as the use of a knowingly forged document in order to obtain certain rights under Part 3 of Art. 327 of the Criminal Code of the Russian Federation.
According to Part 3 of Art. 327 of the Criminal Code of the Russian Federation may also qualify the actions of drivers in providing the traffic police with knowingly forged purchased medical reports produced by fly-by-night companies and issued without passing all the required examinations.

Responsibility for violations
If the employee has not undergone a medical examination and has not been removed from his duties, the employer will be responsible for this. For each such employee, considerable fines are provided:
